after unlocking is it permanently or trial? Because one of my fellow experience the same problem. after he unlocked his modem but not for this software, after two months the modem is not working. it is dead for real

Unlocking with DC-Unlocker is permanent - once modem is unlocked, it is unlocked forever. Firmware change / flashing modem may relock it.
Unlocking same device with same username does not consume credits again.
