Has my E5372T been unlocked?

I purchased an unlock code from you. I think I went through the steps correctly. Is my modem unlocked now?? See below.

I wrote down the 8 digit code. Can I enter this in again manually? I can't see anywhere to do that.

If not how can we fix this?
