E160 problem, need help

i read that it is supported and on one forum somebody sad to me that this program is working with that card and operater...
operater is T-mobile hr

ok, i have installed something called dashboard and now it's sending me a message no username...
where do i get username and pass for server login in that program???
and how much does it cost???
