What is wrong, please help

Be sure all drivers are installed.
Check, if no other modems or phones are connected to computer at unlock time.
Also, check, if you do not miss to reconnect usb cable when software ask for it.
Also you can try unlocking again. Unlocking same device again does not require credits.

Fully install the driver, unlock the process, the computer prompts to re-insert the USB cable, but still wrong unlock

Try to check, if you see any unknown devices after error. If so, then update the unknow device with drivers from phone or software folder. Then do unlock again.
