Try to use this firmware: http://www.dc-unlocker.com/test/GTM3...v1.4.6.0Hd.zip
Then enable all bands. After you can upgrade to any firmware you wish to have

      Currently the firmware 2.9.4.11Hd
      Status already SIM Unlocked.

If I downgrade to above firmware as your advice, will it be SIM locked?

Where can I find the the latest firmware? I can not find it at Option website.

      See the attached snapshot.

There is no other way to check, only to test with few providers that use other bands (frequencies).
If you are able to use your modem with various providers then all bands are enabled.

I find the GTM382 has serval type for various providers ,they have different frequency definited, mo0401 just support 900 2100Mhz; mo0402 support 850/1900/2100Mhz; m0407 support 850/1900/2100Mhz/AWS.
I want to know does the Mo0401 enable all bands support 850/1900Mhz

Sorry, can not tell for sure. Enabling all bands enable all frequencies that are supported from manufacturer.
