Sierra AC754S have code, what next?

I contacted AT&T and got an unlock code, but I'm not sure how to enter it into the device. Where/how do I insert this code to unlock the hotspot?

FYI, I tried running the software, but it asks for a username and password when I click on unlock. not sure what it's looking for there.

Sorry, dc-unlocker doesnot have function to enter unlock code for these device. We only provide unlock by using dc-unlocker software.
